  • Business Debt Schedule
  • 2 Years Business Tax Returns Years Personal Tax Returns 
  • 3 -6 Months Business Bank Statements or Bank Verification 
  • YTD Profit & Loss and Balance Sheet 
  • Personal Financial Statement
  • No Liens 
  • No Foreclosure 
  • No open Tax Liens
  • Government Issued ID (i.e. Driver's License)
  • SBA Form 413
